The Connected Community Care service connects people to local groups, services, and activities to support their wellbeing.
Services:
If you are aged 18 or over and live within the Greater Belfast area BT1 – BT17
- Citywide social prescribing service – collaborates across the health service, with other statutory providers, and with community and voluntary organisations to connect people at risk of developing chronic conditions to early intervention and prevention initiatives, to support with self-management, to tackle social isolation and loneliness, to provide pre and post diagnosis support for dementia and to link people living with cancer to support services within their local community.
- Wellbeing Co-ordinators – Specialises in supporting anyone who is at risk of or has been diagnosed with a chronic health condition and/or social isolation.
- Dementia Navigators – Providing pre-diagnostic information and support for people who have been referred for a memory assessment by their GP and are waiting for an appointment or/and are post diagnostic. Information and support is also offered to carers.
- Connected Community Care for Cancer – The Macmillan Community Link Workers specialise in supporting people affected by cancer and provides information and connections to support services. Due to the connection with the NI Cancer Centre located at Belfast City Hospital this is now a regional service.
